Cub Scout Pack 375 Forms

 

Below are forms you may need throughout the Scouting year.  Please contact the Cubmaster with any questions!

Youth Application & Membership Order Form
 

New Scouts are encouraged to apply using BSA's on-line application tool.  Please contact our Cubmaster at cubscouts375@gmail.com for a link and additional joining information.  

 

Returning Scouts do not need to fill out another application.  

 

The Membership Form below is used to calculate annual dues, order handbooks handbooks, and to subscribe to Boys Life Magazine.

Adult Applications

 

Adults wishing to volunteer with the Pack must submit a paper copy of the BSA Adult Application due to the additional documentation required by Pennsylvania Act 15 (see below).  Please do not miss the Background Check Authorization near the end of the application.

Pennsylvania Act 15 requires that adults (those 18 years of age or older) having direct volunteer contact with children will need background check clearances. Therefore the following documents are required with your Adult Application:

  1. PA Child Abuse History Clearance

  2. PA State Police Criminal Record Check

  3. FBI Criminal Background Check or Self Disclosure Form

These documents are explained very well on the State College Area School District Website.  

​

In addition, BSA requires that all registered adults must complete their Youth Protection Training.  This an on-line course that takes approximately an hour.  You can access the course by logging into your my.Scouting account.  Once logged in, use the drop-down menu to select "My Dashboard" and from there select "Training."  Youth Protection Training (YPT) will be the first option.  Once the YPT is complete, you will have the opportunity to print a completion certificate.  Please attach a copy of the certificate to your application.

Request for Reimbursement Form

 

Please use this form to request reimbursement for your approved Cub Scout Den or Pack purchases.  Items that are reimbursable are Den/Pack meeting supplies, food for Den/Pack meetings, events, and functions, and Pack Leadership training costs.  If you have any questions about whether your expenses are reimbursable, please contact the Cubmaster.
